A STUDY ON THE FACTORS AFFECTING TRANSFER AMONG CLINICAL NURSES IN NIGERIA

CHAPTER ONE

INTRODUCTION

Background of the study

The phenomena of clinical nurse transfers across healthcare systems is noteworthy and is impacted by a multitude of factors that affect patient care outcomes in addition to the workforce. The term "nurse transfer" describes the movement of nurses between various healthcare facilities or within the same organisation. It is imperative to comprehend the variables influencing this transfer because it can have significant effects on nurse retention, work satisfaction, and the provision of healthcare as a whole.

Numerous organisational and individual factors that affect nurse transfer have been found in recent studies. For instance, nurses' decisions to transfer are greatly influenced by the organisational culture, which includes mentoring programs and management assistance. A positive workplace culture influences the chance of transfer by increasing job satisfaction and decreasing intention to leave (Kang et al., 2022). Moreover, nurses frequently cite the possibility of professional growth and career promotion as key drivers for moving to new departments or organisations (O'Leary et al., 2023). These kinds of chances help them achieve their long-term professional goals in addition to improving their skills.

Furthermore, nurses' decisions to transfer are greatly influenced by individual factors such as work-life balance, stress connected to the profession, and personal situations. Nurses who experience high levels of job stress and burnout may look for transfers as a way to relieve these demands (McGillis Hall et al., 2023). Furthermore, nurses frequently think about moving to units that better suit their needs due to the need for a more flexible schedule or a change in personal circumstances, such as family obligations (Fitzgerald et al., 2024).

The interaction of these variables emphasises how difficult it is to decide which nurses to move, and how healthcare administrators must put plans in place that take organisational support and the needs of specific nurses into account. Healthcare organisations can raise the standard of patient care overall and increase nurse retention by addressing these criteria (Buchan et al., 2023).

Having a thorough grasp of the factors affecting clinical nurses' transfers is crucial to creating retention plans that work. Recognising and addressing these issues will be essential to keeping a stable and content nursing workforce as healthcare systems encounter more and more difficulties. In the end, this will benefit patients as well as nurses.

 Statement of the problem

Many variables that have a major impact on patient care, staff retention, and nursing practice affect the transfer of clinical nurses within Nigerian healthcare facilities. Even though transfers are essential for improving workforce distribution and addressing staffing shortages, a number of factors influence the processes involved in making decisions about nurse transfers. Organisational policies, work satisfaction, possibilities for professional advancement, and the socioeconomic climate are important elements that jointly influence nurses' attitudes and views regarding transfers.

According to recent studies, organisational regulations frequently control the transfer procedure, which makes nurses feel unfairly treated and dissatisfied (Adebayo & Oladejo, 2023). Furthermore, nurses' concerns about receiving insufficient support and recognition in their existing roles have been recognised as a significant factor of job satisfaction, which in turn affects their readiness to accept transfers (Ogunleye & Afolabi, 2022). Furthermore, as nurses look for transfers that fit with their career goals and skill development, the quest of professional progress continues to be an important factor (Chukwu & Ogbonna, 2023). The transfer dynamics are further complicated by the socioeconomic issues that are common in Nigeria, such as poor infrastructure and low pay, which frequently result in higher turnover rates among clinical nurses (Nwankwo et al., 2023).

Given these difficulties, it is imperative to look into the complex variables influencing nurse transfers in Nigeria. Gaining an understanding of these components can help improve workforce management techniques and, eventually, the results of healthcare delivery.

1.3 Objectives of the Study

The primary objective of this study is to examine the factors affecting transfer among clinical nurses in Nigeria. Specifically the study seeks:

To identify the key factors influencing the transfer decisions of clinical nurses within healthcare institutions in Nigeria.

To examine the impact of factors on the transfer decisions of clinical nurses in Nigeria.

To propose recommendations for improving transfer processes and policies to enhance nurse retention and job satisfaction in the healthcare system in Nigeria.

1.4 Research Questions

The following research questions will be answered in this study:

What are the key factors influencing the transfer decisions of clinical nurses within healthcare institutions in Nigeria?

What are the impact of factors on the transfer decisions of clinical nurses in Nigeria?

What are the recommendations for improving transfer processes and policies to enhance nurse retention and job satisfaction in the healthcare system in Nigeria?

1.5 Research Hypothesis

The stated hypothesis will validate this study:

Ho1: There is no significant factors affecting transfer among clinical nurses in Nigeria.

Ha1: There is a significant factors affecting transfer among clinical nurses in Nigeria.

1.6 Significance of the study

First and foremost, this study is essential to the development of policies for the Nigerian healthcare system. Through the identification of factors that impact nurse transfers, policymakers might devise more efficacious approaches to maintain competent nursing personnel in crucial regions. This is beneficial for keeping qualified nurses on staff as well as guaranteeing patients receive continuous care, which is essential in clinical settings.

Furthermore, this study can advance knowledge of the dynamics of the nursing workforce. The study aims to investigate the ways in which organisational culture, job happiness, career development prospects, and personal circumstances interact to impact nurses' decisions to transfer. With this knowledge, nursing managers and leaders may create customised strategies for staff engagement and retention that will increase the stability and experience of their nursing team.

First and foremost, the creation of policies for the Nigerian healthcare system depends on the results of this study. By identifying the variables that affect nurse transfers, governments could develop more effective strategies to keep skilled nurses in key areas. This is advantageous because it ensures that patients receive ongoing care, which is crucial in clinical settings, and keeps qualified nurses on duty.

Additionally, this research can contribute to our understanding of the dynamics of the nursing workforce. The purpose of the study is to ascertain how nurses' decisions to transfer are influenced by the interplay of organisational culture, job satisfaction, career development opportunities, and personal circumstances. Equipped with this knowledge, nursing managers and leaders can devise tailored approaches for engaging and retaining people, thereby augmenting stability and experience of the nursing team.

1.7 Scope of the study

The study aims to examine the factors affecting transfer among clinical nurses in Nigeria. Empirically, this study assesses the key factors influencing the transfer decisions of clinical nurses within healthcare institutions in Nigeria, the impact of factors on the transfer decisions of clinical nurses in Nigeria and propose recommendations for improving transfer processes and policies to enhance nurse retention and job satisfaction in the healthcare system in Nigeria.

1.8 Limitation of the study

Like in every human endeavour, the researchers encountered slight constraints while carrying out the study. The significant constraint was the scanty literature on the subject owing that it is a new discourse thus the researcher incurred more financial expenses and much time was required in sourcing for the relevant materials, literature, or information and in the process of data collection, which is why the researcher resorted to a limited choice of sample size. Additionally, the researcher will simultaneously engage in this study with other academic work. However in spite of the constraint all these constraint were downplayed to give the best.

1.9 Definition of terms

Nurses: Nurses are healthcare professionals trained to care for individuals, families, and communities to help them achieve and maintain optimal health. They provide medical care, administer medications, monitor patient conditions, and offer support and education to patients and their families.

Factors: Factors are the elements or circumstances that contribute to a particular result or situation. They can influence outcomes in various contexts, such as in decisions, behaviors, or processes.
